When do I use this form?
The Payment Request form should be used to reclaim money for individuals using receipts. You should not be reclaiming more than £100 for each person at a time.

You should also use this form to request payment for Club coaches and instructors using invoices.

Who do I return it to and what should I attach?
The Payment Request form needs to be submitted as a hard copy to the CSC Reception. All the receipts that you are reclaiming for should be attached to the form.

What happens after I have submitted it?
The Union pays money back directly to individual bank accounts every Friday. The money should turn up in the payee’s account on the following Wednesday.
